Access Your Health Information Online

MyChart

If you have an active MyChart account already in use, you may login to this account. You will note there is information displayed to navigate you to the new provincial patient portal, ConnectMyHealth.

If your account is inactive, or you are attempting to register another user, MyChart is no longer available in Southwestern Ontario.

ConnectMyHealth Portal

ConnectMyHealth is a digital health solution that provides you with an online, single access channel to view your health records from many healthcare facilities in Ontario. ConnectMyHealth can be used on desktop and mobile devices from within Canada, there’s no cost to use it, and there’s no app to download when accessing your account via the web.

ConnectMyHealth is different than other patient portals because it combines various types of health records created at hospitals you’ve been to in the Ontario Health West Region into one, consolidated view. Additional data sources, like community-based laboratories and radiology clinics, are expected to be added in the near future. ConnectMyHealth can be an ideal complement to in-person or virtual visits with your care providers by providing you easy access to your hospital records, and to additional functionality you may have through your local hospital, like appointment scheduling.

Temporary Emergency Department Closure

Wingham and District Hospital

As a result of a staffing shortage, the Wingham and District Hospital Emergency Department will be temporarily closed this weekend as follows:

  • Friday June 9th from 5 pm until 7 am the following morning
  • Saturday June 10th from 5 pm until 7 am the following morning
  • Sunday June 11th from 5 pm until 7am on Monday June 12th.

Patients are asked to call 911 if they have a medical emergency. Emergency Medical Services (EMS – Ambulances) will remain available to the community and will be re-routed to surrounding hospitals to ensure local residents continue to have access to the emergency services they need.

For non-urgent health care call Health Connect Ontario at 811, which is available 24/7 for non-emergency health-related questions.

The following is a list of closest hospitals (they may also be experiencing temporary closures due to staffing shortages so it is advised that you confirm that they are open before heading there):

  • Listowel
  • Clinton
  • Goderich
  • Palmerston
  • Seaforth
  • Kincardine
  • Walkerton

This is a temporary measure and our staff, board, physicians and leadership regret that we have to take this step.
