Access Your Health Information Online Through MyChart
What is MyChart?
MyChart is a secure online service that gives you, or your substitute decision maker, access to your medical records, from various participating hospitals.
How do patients access MyChart?
MyChart is accessible from anywhere at any time through the internet. Once registration is complete users can go to www.mychart.ca and log in to review their MyChart records.
What do patients have access to through MyChart?
Currently the data available in MyChart is hospital-based and, at this time, does not include information from your primary care provider’s/family physician’s electronic medical record.
Available hospital-based information includes:
- Medication lists
- Laboratory tests
- Blood bank results
- Radiology reports
- Pathology reports
- Microbiology results
- Genetics results
- Discharge summaries
Some data availability is delayed due to the potential sensitivity of the data and/or the need for clinical review and/or quality assurance, prior to disclosure.
How do I sign up?
To sign up, go to www.mychart.ca and click Register.
You can also register for MyChart in person at either Listowel or Wingham hospital at the main registration areas or through the Health Records departments.
- You, or your substitute decision maker will be required to provide an Ontario health card and a valid email address
- Immediately after providing your email, Sunnybrook Health Sciences will send you an email from the email address “email@example.com” with a link to www.mychart.ca and a four digit PIN to complete registration
Can minors register for a MyChart account?
Yes, everyone with an Ontario health card is eligible for a MyChart account. If you are under 14, your parent or legal guardian will be responsible for registering and managing your account on your behalf (unless requested otherwise*). At 14 years old, you will re-register your account and take ownership as account manager, unless otherwise noted based on capacity assessment.
*Children 12 and over may register for their own account if deemed appropriate based on capacity and requested by the child. All children 12 and over must be present at the time of registration for a MyChart account. Children under the age of 12 may be fully registered by a parent or legal guardian.
